This form should be used to request a loan from your Academy. This will typically be for Season Ticket loans, however in exceptional circumstances you may request advances for other types of loan (e.g. support with CPD costs), which may or may not be approved at the absolute discretion of the Trust. 

Once you have completed the form, you will then need to submit to your Local Head of Finance and Principal/Headteacher for approval.
